1) Family Medicine Approach to Complex Needs Course – 19 September 2019 Thursday
Complex patients are increasingly common in primary care and often have poor clinical outcomes. Healthcare system barriers to effective care for complex patients have been previously described, but less is known about the potential impact and meaning of caring for complex patients on a daily basis for primary care providers (PCPs). Family Medicine Approach to Complex Needs Course objective is to describe PCPs’ experiences providing care for complex patients, including their experiences of health system barriers and facilitators and their strategies to enhance provision of effective care.
Target Audience: Primary Care Physicians and health workers of all ages, from all career levels. We expect that family doctors will find the course to be challenging and applicable.

2) Population-Based Health Promotion and Disease Prevention Course – 22 September 2019 Sunday
Health promotion and disease prevention are a major emerging theme in geriatric medicine and health care generally. Although efforts have typically been targeted at younger persons, there is growing evidence that this approach is both appropriate and feasible for those age 65 and over.
Course objective is defining a major goal of the health promotion and disease prevention approach—both for individuals and for an entire population— to identify the health problems for which preventive efforts can result in more appropriate utilization of health services and improvements in health status.
